Membership Agreement

If my membership application is accepted, I agree to abide by all rules, regulations, policies, and expectations of New Line Boxing Academy. I understand that failure to comply with these rules, regulations, policies, and expectations may result in immediate disciplinary action, up to and including membership termination.

Rules, regulations, and policies are subject to change.

Furthermore, I understand that before participating in any physical activities, exercise programs, or activities of that nature, I must consult a physician for medical approval.

I understand that classes falling on statutory holidays may be cancelled with or without prior notice.

Waiver

Please read completely and carefully. If you have any questions, consult with New Line Boxing Academy officials before accepting this waiver.

The risk of injury from the activities involved in these boxing programs is significant, including the potential for permanent paralysis and death. While particular rules, equipment, and personal discipline may reduce these risks, the risk of serious injury still exists; and,
I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of New Line Boxing Academy, its officers, agents, officials, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessees of the premises used to conduct the event (collectively, the “Releasees”), or others, and I assume full responsibility for my participation; and,
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless the Releasees with respect to any and all injuries, disability, death, or loss or damage to person or property, whether caused by the negligence of the Releasees or otherwise, including but not limited to:
(a) breach of any other duty imposed by law, including any duty imposed by occupiers’ liability or other legislation;
(b) breach of any contract; and
(c) mistakes or errors in judgment of any kind.
In accepting this Agreement, I am not relying on any oral, written, or visual statements of any kind, including promotional statements made by New Line Boxing Academy, its brochures, or any of the Releasees to induce me to participate in New Line Boxing Academy’s programs.

I have read this Release of Liability and Assumption of Risk Agreement, fully understand its terms, understand that I am giving up substantial rights by accepting it, and accept it freely and voluntarily without any inducement.

For Participants Under the Age of 18 Years Old

I certify that I am the parent or guardian with legal responsibility for this participant and consent to and agree to their release as provided above with respect to all Releasees.

For myself, my heirs, assigns, and next of kin, I release and agree to indemnify the Releasees from any and all liabilities arising from or related to my minor child’s involvement or participation in New Line Boxing Academy programs as provided above.

Consent for Release of Information

New Line Boxing Academy requires permission in order to post a registrant’s information and/or photos on its website, in newsletters, or in any other public domain.

We respect and protect the privacy of our registrants. Personal information is used only for boxing-related purposes. Examples of where information or photos may be used include:

The use of a registrant’s name and/or photo in New Line Boxing Academy newsletters, on the New Line Boxing Academy website, on bulletin boards, or in local newspapers.
The taking of individual or team photos and the use of an athlete’s name and/or photo in boxing programs and related materials.
The circulation of information promoting boxing-related information and opportunities.

By accepting this waiver, I give consent for New Line Boxing Academy to use information and/or photos for the purposes specified above.
