WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT

1. ACKNOWLEDGMENT OF RISK

I acknowledge that participation in basketball training programs, including but not limited to private training, group sessions, camps, strength and conditioning, drills, scrimmages, and related activities, involves inherent risks.

These risks include, but are not limited to:

Sprains, strains, fractures, and dislocations
Head injuries, concussions, and serious bodily injury
Muscle, ligament, or joint damage
Slips, falls, and collisions with other participants or equipment
Injuries arising from use of gymnasium facilities, courts, or training equipment

I fully understand that these risks may arise from my own actions, the actions of others, or the conditions of the training environment, including gymnasiums and training facilities.

2. ASSUMPTION OF RISK

I voluntarily choose to participate in programs offered by Elvis Dennis Training Academy, its affiliated coaches, trainers, staff, contractors, and facility providers.

I freely accept and fully assume all risks, both known and unknown, even if arising from negligence, related to participation in any training activity.

3. RELEASE OF LIABILITY

In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless:

Elvis Dennis Training Academy
Elvis Dennis and all affiliated coaches, trainers, and staff
Any partner organizations, facilities, gyms, or locations used
All employees, contractors, volunteers, and representatives

from any and all claims, demands, actions, damages, losses, or liabilities, including but not limited to:

Personal injury or death
Property damage or loss
Injuries sustained during exercise, gameplay, or gymnasium use

This release applies whether caused by negligence or otherwise, to the fullest extent permitted by law.

4. MEDICAL CONSENT

I certify that I am physically fit to participate in athletic training.
I understand that it is my responsibility to consult a physician prior to participation if necessary.

In the event of injury or emergency, I authorize staff to:

Administer basic first aid
Seek medical attention on my behalf

I accept full responsibility for any medical costs incurred.

5. FACILITY & EQUIPMENT USE

I acknowledge that training may occur in various gymnasium settings and facilities, and I accept all risks associated with:

Court surfaces
Equipment use (balls, weights, agility tools, etc.)
Facility conditions

6. INDEMNIFICATION

I agree to indemnify and hold harmless all parties listed above from any claims brought by or on behalf of myself, including legal fees and costs.

7. PHOTO & MEDIA RELEASE

I grant permission for Elvis Dennis Training Academy to use photographs or videos taken during training for promotional purposes.

8. GOVERNING LAW

This agreement shall be governed by and interpreted in accordance with the laws of the Province of Ontario, Canada.

9. SEVERABILITY

If any portion of this agreement is found to be invalid, the remaining provisions shall remain in full force and effect.

10. ACKNOWLEDGMENT & SIGNATURE

I confirm that I have read and fully understand this agreement. I am aware that by signing, I am waiving certain legal rights, including the right to sue.
