As parent/legal guardian of the child named herein, I hereby represent that the child has been examined by a pediatrician and is physically fit to participate in Coaching Volleyball Champions/Lethbridge Volleyball Club camps. I understand there are inherent risks to participating in this athletic program. I hereby accept responsibility for and agree to pay any and all costs of medical treatment resulting from any injury suffered by my child as a result of his/her participation at Coaching Volleyball Champions/Lethbridge Volleyball Club. I further agree to indemnify and hold harmless Coaching Volleyball Champions/Lethbridge Volleyball Club and its agents, employees and/or representatives, from any and all liability, damage, or expense arising out of my child’s participation at Coaching Volleyball Champions/Lethbridge Volleyball Club. In the event that I cannot be reached in an emergency, I hereby give permission for a qualified Coaching Volleyball Champions/Lethbridge Volleyball Club staff member, an emergency medical technician, a physician or staff member at a hospital, or any other qualified individual to administer care and provide any medical treatment deemed necessary for my child. I understand that checking this box constitutes a legal signature confirming that I acknowledge and agree to the above terms. As parent/legal guardian of the child named herein, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child may be exposed to or infected by COVID-19 by attending activities at Coaching Volleyball Champions/Lethbridge Volleyball Club all and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Coaching Volleyball Champions/Lethbridge Volleyball Club may result from the actions, omissions, or negligence of myself, my child, and others, including, but not limited to Coaching Volleyball Champions/Lethbridge Volleyball Club’s employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or my child (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my child’s attendance at Coaching Volleyball Champions/Lethbridge Volleyball Club camps or participation in the programming (“Claims”). On behalf of my child I hereby release, covenant not to sue, discharge, and hold harmless Coaching Volleyball Champions/Lethbridge Volleyball Club, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions or negligence of Coaching Volleyball Champions/Lethbridge Volleyball Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Attack Volleyball program. I understand that checking this box constitutes a legal signature confirming that I acknowledge and agree to the above terms. I, hereby grant and authorize Coaching Volleyball Champions/Lethbridge Volleyball Club the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of my child to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, websites, social networking, and other digital and print communications, without payment or other consideration. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. I understand and agree that these materials shall become the property of Coaching Volleyball Champions/Lethbridge Volleyball Club and will not be returned. I hereby hold harmless, and release Coaching Volleyball Champions/Lethbridge Volleyball Club from all liability, petitions, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons may make while acting on my or my child’s behalf.

Coaching Volleyball Champions/Lethbridge Volleyball Club Refund Policy

Citrus Processing Fee is Non-Refundable.
Event Fees: Prior to 30 days advance of a Coaching Volleyball Champions/Lethbridge Volleyball Club event starting we will offer a full refund minus a $25.00 administration fee and applicable taxes.
Less than 30 days to the start of camp, we do not offer refunds. Coaching Volleyball Champions/Lethbridge Volleyball Club reserves the right to allow for exceptions to this for medical and approved circumstances by ATTACK Volleyball. If approved, we will offer camp credit minus a $25.00 administration fee.
