Mandatory form for all new customers, please complete. Completed information gives us insight on your child’s level of play, experience, and uniform size.

Uniform T-Shirt $ 19.99 plus taxes.

Uniform Socks $ 14.99 plus taxes.

We will email you an invoice for your child’s uniform. Payment can be made via the online link with the invoice or at the academy front desk. Payment must be received before uniform can be picked up.
