Parental Consent and Liability Release

Dear Parent or Guardian,

Please read and complete this waiver form for your child’s participation in AirJam Sports activities.

Your acceptance indicates your understanding and agreement to the terms below.

Waiver and Release

I, the undersigned parent or legal guardian, hereby give permission for my child to participate in AirJam Sports

programs and events. I acknowledge that participation involves physical activity and inherent risks. I release AirJam

Sports, its staff, coaches, and affiliates from any liability for injuries or damages that may occur during these

activities, except in cases of gross negligence or intentional misconduct.

I certify that my child is physically fit and able to participate. I agree to notify AirJam Sports of any relevant

medical conditions or concerns prior to participation.

Medical Consent

In the event of an emergency, I authorize AirJam Sports staff to provide or arrange for medical treatment for my child and to contact the listed emergency number.
