ASSUMPTION OF RISK
I, the undersigned participant (or parent/guardian if the participant is a minor), acknowledge that participation in Stutter Steps Basketball activities—including but not limited to training sessions, team practices, scrimmages, and tournaments—carries inherent risks of physical injury, including but not limited to falls, collisions, and other sports-related incidents. I voluntarily assume all such risks associated with my/my child’s participation.

RELEASE OF LIABILITY
In consideration of being allowed to participate in Stutter Steps Basketball programs, I hereby release, waive, and discharge Stutter Steps Basketball, its directors, coaches, trainers, volunteers, sponsors, and facility owners from any and all liability for personal injury, property damage, or wrongful death arising from participation in any of its activities, whether caused by negligence or otherwise.

MEDICAL AUTHORIZATION
I understand that in the event of an injury, every reasonable attempt will be made to contact me (or the emergency contact listed). If I cannot be reached, I authorize Stutter Steps Basketball and Stutter Steps Hoop Club staff to seek and provide emergency medical treatment as deemed necessary.

INSURANCE & RESPONSIBILITY
I affirm that I/my child am/is physically fit and able to participate in basketball activities. I acknowledge that I am solely responsible for securing any necessary medical or accident insurance coverage, and that Stutter Steps Basketball and Stutter Steps Hoop Club do not provide such coverage.

I understand that optional insurance coverage may be available through Stutter Steps Basketball’s membership with Basketball Saskatchewan Inc.; however, any insurance-related matters are to be handled directly between the participant (or their parent/guardian) and Basketball Saskatchewan Inc. Stutter Steps Basketball and Stutter Steps Hoop Club bear no responsibility or liability for the provision, denial, or management of such insurance coverage.

MEDIA RELEASE
I grant permission for Stutter Steps Basketball to use photos or videos of me/my child for promotional or educational purposes, including on social media, websites, and marketing materials. I understand that no compensation will be provided.
